This Registered Nurse position will work at LifeWorks NW's Certified Community Behavioral Healthcare Clinics (CCBHC). This position will work directly with LifeWorks NW's Assertive Community Treatment (ACT) Team.  This team provides community based services for clients with severe and persistent mental illness and who are unable to benefit from a traditional office based approach.

Purpose:  The Registered Nurse (RN) is primarily responsible for medication management, education and support, assisting clients in managing medical conditions including metabolic syndromes and assisting clients in meeting health care needs in the community setting.

Hours: This is a full time position, will be required to assist with the ACT Crisis Line one time per month (additional stipend given) 

Rate of Pay:  $75,626 - $83,189/per year with a $2,000 sign on bonus

Essential Responsibilities:

  • Responsible for all medication management, education and support, including working closely with LMP and local pharmacies to ensure accurate medication ordering and refills, giving injectable medications and being current and up to date with knowledge surrounding psychotropic medications. These services may be completed in the clients' home.
  • Completes health screenings (i.e. tobacco use, BMI, blood pressure, diabetes, etc) and works closely with client Primary Care Providers to assist clients in managing and monitoring these conditions.
  • Reviews lab work such as diabetes screenings, blood levels related to psychiatric medications and follow-up as needed.
  • Helps to develop multi-disciplinary wellness plans
  • Identifies primary care physicians or assists clients in establishing a primary care provider.
  • Follows up with physicians to ensure communication is achieved within specified timeframes.
  • Communicates and explains the procedures to prepare clients for examinations and treatments.
  • Helps monitor general health symptoms, side effects, and differentiates physical illness from exacerbation of psychiatric symptoms.
  • Assures that required labs are completed on an ongoing basis.
  • Provides wellness education to individuals and families. 
  • Continually increase level of cultural sensitivity, awareness and competency.
  • Establishes and maintains positive and professional working relationships with co-workers, clients, and other agencies.
  • Provides back up and support to co-workers in providing assistance during medical or mental health emergencies.
  • Meets standards for documentation and accuracy of work, client care, quality/customer service, attendance/punctuality and teamwork as defined in the Performance Evaluation.
  • Enters all required data into agency specific electronic health records system and program specific tracking systems, provides regular reports as requested, and continually maintains all data on clients enrolled in the program.
  • Is available to clients during work hours and for consultation to clinicians who need crisis information after work hours.
  • Transports clients in  personal vehicle (mileage reimbursement provided)


  • Registered Nurse license required.
  • Two or more years of experience working with clients with severe and persistent mental illness strongly preferred.
  • Good computer skills including a functional knowledge of Microsoft Outlook, Word, and Access.  Ability to master proprietary software including electronic health record system.
  • Demonstrates effective communication skills and maintains a cooperative working relationship with others in a culturally diverse environment.
  • Ability to transport clients in personal vehicle, must have access to car, valid driver's license, proof of current automobile insurance coverage, and verification of safe driving record from DMV.
  • Ability to perform essential functions of job without creating a direct threat to the safety of self or others.
