The Recovery Mentor position will work at LifeWorks NW's Certified Community Behavioral Healthcare Clinic (CCBHC) in Gresham, OR. 

In 2017, LifeWorks NW was selected to represent Oregon in the federal demonstration project to implement CCBHCs to advance delivery of behavioral health care. LifeWorks NW's CCBHC sites provide comprehensive client care through a team-based approach with services that include mental health and substance use counseling, peer-delivered health and wellness services, primary care services and psychiatric rehabilitation. Staff that work at our CCBHC sites collaborate with others to care for individuals through daily interactions and support, counseling, skills training, care coordination, health promotion and education, medication management services and supportive services with preventive approach.

PURPOSE: Position provides peer support and a variety of recovery-oriented services to individuals. Peer delivered services include a combination of case management, referral linkage, individual counseling, family support, group counseling and skill building services to assist client's successful engagement in treatment.

Services provided include a wide variety of supports, services and advocacy that contribute to a client's ability to engage in ongoing treatment and work towards a lower level of care. These activities acknowledge the importance of involving significant others and other natural supports in the persons treatment that could positively impact their recovery. and the family's ability to support the client's continued sobriety and recovery. This particular positon will work with adults and youth. 

We are looking for a candidate who is personally in recovery for this position (at least 3 years clean and sober). 

DAYS/HOURS: This is a full-time position working 40 hours per week. Must be able to work daytime and evening hours. 

PAY/BENEFITS: $16.05 to $17.25 per hour. Full benefits.

ESSENTIAL RESPONSIBILITIES:

  • Screens and selects clients to participate in the mentoring program.
  • Develop the service plan for all clients including a service reporting plan on each referral.
  • Within the first five working days following intake into services, the client and (if appropriate) family members meet with the Recovery Mentor and the Primary Counselor to develop a case management plan.
  • Should have daily contact with clients for up to the first five weeks after release helping with engagement in recovery support, housing and employment issues, family adjustment issues, legal, housing, medical and benefit issues. Frequency of contact should gradually decrease to the point of twice monthly or monthly contact by the ninetieth day after release.
  • Responsible for assisting the family in the initial implementation of the plan, maintaining regular communications with the client's primary therapist, collateral providers, community corrections staff, and linking the client system to needed services within the community.
  • Determines when to terminate clients from the program because services are no longer necessary or the client is not responding to the mentor model.
  • Meets regularly with staff from social service agencies to update them on LifeWorks NW services identifying unmet services/needs LifeWorks NW may be able to address; review and update referral and case coordination procedures as needed; obtain referral requests and work with referral sources to remove any barriers to treatment; report back if clients fails to attend; obtain relevant case information from the referral for the clinician who will be doing the client's intake assessment; work with referral source and client to facilitate the family's involvement in the assessment and the treatment planning process.
  • Meets standards for documentation and work accuracy including timely completion of progress notes, assessments, treatment plans, and authorization requests.
  • Meets standards for direct service, implementation and utilization of the clinical model, quality of clinical practice and teamwork.
  • Transport clients in a personal vehicle to meetings/appointments within LifeWorks NW and within the community.

QUALIFICATIONS:

  • Must be a Certified Recovery Mentor or have completed a Recovery Mentor approved training (see this page of the ACCBO website: http://www.oregon.gov/oha/amh/pd/Pages/approved-training.aspx).
  • Must be able to provide required letter from the Office of Equity and Inclusion (OEI) with the Traditional Health Worker Number. If interviewed, must be able to provide a copy of the letter.
  • If recovering from a substance abuse related disorder, must be able to demonstrate continuous sobriety under nonresidential, independent living conditions for the immediate past three years.
  • Substantial experience with issues involving addiction including the language, values, norms and other attributes of the subculture; the subtleties and linguistic nuances of the differences between honest dialogue and statements made for effect.
  • Extensive knowledge of how to efficiently navigate the social service network from the bottom up.
  • Must be able to accompany clients throughout their environment.
  • Must have knowledge of the legal/corrections system.
  • The ability to maintain confidentiality and appropriate boundaries.
  • Demonstrates effective communication skills and maintains a cooperative working relationship with others in a culturally diverse environment.
  • Basic computer skills including keyboarding, use of mouse, ability to navigate website and use common web browser.
  • Position must be able to drive clients in a personal and a LifeWorks NW vehicle. Must have a valid driver's license. current auto insurance, and ability to meet LifeWorks NW driving requirements.
